The Public Liability (Property Damage) form is used to make a claim if your property
is damaged because of the negligence of the Department, or because it has not carried out its duties
properly.
Completing a claim form will assist Central Claims Unit to process
your claim as quickly as possible. You may request a claim form from either of the Unit's two Offices
either via e-mail, by telephone or in writing.
