60+ SmartPass - Completing my Application Form

1. When you receive your application form, read the guidance notes attached to the form as these contain all the information you need to complete your form correctly.
2. Complete the “Personal Details” section of the application form.
   
3. Read and sign the Declaration.
   
4. Have your countersignatory (someone who knows you and is not a relative) complete their details and sign your application form and a recent, colour, passport-type photograph of you.
   
5. Attach the photograph to the application form.
   
6. Take your completed application form, along with proof of age and residence, to the relevant application centre.
   
7. Your SmartPass will be posted direct to your home address.
